Documents and Forms for YOBC 2022 International Tour

Required Forms

In addition to the tour application form, there are a number of forms that all tour participants will be required to complete.  All forms will be available on the tour page of the YOBC website. Most forms can be submitted electronically.  Please take care to submit all forms by their due dates.

Please complete and submit the online forms below by the deadlines indicated. Most of the forms require both student and parent/guardian signatures, so please take the time to complete them together.

1. PASSPORT LOG FORM

Please fill out a separate Passport Log Form for each traveler in your party.

Due ASAP but no later than February 18, 2022.

Mail 3 copies of each passport to our passport coordinator:

2. INSTRUMENT INFORMATION FORM

You will need to know:

• the maker and value of your instrument/bow/case;
• your insurance policy number and expiration date;
• the dimensions and weight of your instrument and case.

Form must be signed by student/musician AND parent/guardian.

3. MEDICAL/HEALTH FORMS

Health Form

All YOBC students must submit a Health Form completed and signed by a parent. This information will be treated confidentially and will be reviewed and held by the doctors who will be traveling with the tour group and providing medical support. Adults who are participating in the tour and wish our medical support team to have access to their health information may choose to submit a form as well. Please submit a separate form for each traveler.

Medication Form

All YOBC students, whether or not they will be taking prescribed medications while on the tour, must submit a completed and signed Medications Form. This information will be treated confidentially and will be reviewed and held by the doctors who will be traveling with the group and providing medical support.

Dietary Needs Form

All persons traveling with the YOBC tour must return a completed Dietary Needs Form. Please submit a new form for each individual traveler.
